How to use Form Track

Create a type

Info

Create a new type which will be used as a base for templates

  1. Site Administration
  2. Formtrack
  3. Manage Types
  4. Select New Type

Select open/restricted visibility based on the types requirements and name the type accordingly (e.g. Appraisal)

Build a template

Info

Create a new template within the type to start defining the sections and questions

Navigate to Form Track Overview

  1. Site Administration
  2. Formtrack
  3. Manage Types
  4. Select Type
  5. Select Template

The template will assign to the selected type, unless all templates are being viewed in which case select the desired type. Select the visibility according to the templates/creators requirements.

Navigate into the template by selecting its name and start adding sections, followed by adding the questions to the appropriate sections.

Assign a form

Info

Forms are assigned based on the Template, to assign a form

Navigate to Form Track Overview

  1. Site Administration
  2. Formtrack
  3. Overview

Select the New button (The button will always indicate the type name e.g. Appraisal)

Give the form a title, select who is to complete it (assignee or manager), then select who the form/forms are to be assigned to (you are able to select multiple users at once).

Managing Forms

Info

Managers and site administrators are able to manage forms for the Formtrack, Overview page by selecting the type or template they would like to manage. Here they can

  1. See a forms status (Assigned, Completed)
  2. Edit form details (title, who is assigned to complete it)
  3. Delete unwanted forms

Learners are able to manage the forms assigned to them from their profile page.

Completing and viewing forms

Learners who are assigned forms will be notified by the platform's notification system, which will indicate to the learners whether they, or the manager, are responsible for completing the form

Forms for site administrators or managers with access to Form Track will find the forms in their respective type, and template. Alternatively all forms associated with the type can be found by viewing the type in

  1. Formtrack
  2. Overview
  3. Type

Learners and managers can view assigned or completed forms by selecting them from the appropriate locations.

Completed forms can be downloaded and if the user has the correct permissions forms can be edited or deleted.
